Bilingual applications for employment are printed in English with Spanish translation. Three hole punched design allows the forms to be displayed or stored in standard three ring binders. Each form includes spaces for the applicant's name, address, telephone number, social security number and "referred by". The "employment desired" section includes the position, earliest starting date, desired salary and boxes to check whether the applicant is legally authorized to work in the United States, whether they are currently employed and whether they previously applied to your company with a space for where and when. The education section provides boxes for details about the applicant's high school, college, and other school, including location, years attended and graduation status. General information covers special training, skills and military service.
